I make bread (by hand) fairly regularly, so I have some ideas. From what you said, probably the biggest problem was that you used too much flour for the amount of water. Bread dough shouldn't be falling apart when you knead it - it should be supple, springy and a bit tacky. Bread recipes, especially...
